    Your Practice Nurse will do this under #721 & #723

    NB: Nearly all our Nurse Therapists are Credentialed Mental Health Nurses via the ACMHN & under #10956

    Does the person need Mx + Therapy for their MH Care?

    CMHNs provide therapy as well as Mx review, monitoring and reporting accurately & reliably back to you on their presentation & the efficacy of their Mx. Remember our primary field of practice is Psychiatry. We have all added Psychotherapy to provide therapy too.

    GPMP + TCA

    Using a chronic disease management plan & team care arrangements, refer patients to a MHN. Medicare refers to us on this form as 'mental health workers'.

    Enhanced Primary Care Referral

    Up to 5 sessions per calendar year. Where possible, ascribe 5 sessions to the MHN and allow 1 for a psychiatrist review. Most of us work with a psychiatrist who will provide a telehealth assessment as needed.
